At Chrystal Care Ltd, our Outreach Support is all about empowering individuals to live independently while receiving the support they need – right in the comfort of their own homes.
Our small, dedicated team of outreach workers is available Monday to Friday, from 7:00am to 5:00pm (with flexibility to suit individual needs). We bring personalised, compassionate care directly to those who may need a helping hand with everyday tasks—whether short-term or as part of a longer journey toward independence.
We currently offer tailored care packages lasting between two and six weeks, designed to provide practical, hands-on support that makes a real difference. These packages include assistance with a variety of daily living activities, helping individuals maintain their routines, build confidence, and enhance their quality of life.

Our Outreach Support may include assistance with:
- Setting and working toward personal goals
- Strategies for staying safe at home and in the community
- Support to attend local activities and outings
- Building and maintaining relationships with family and friends
- Active participation in community life
- Developing independent living skills (e.g. budgeting, cleaning, shopping)
- Enhancing social and communication skills
- Finding and maintaining employment or voluntary roles
- Personal care, meal preparation, and other in-home support as needed
